Download the attached 'livescan" form. Complete your portion and find your local UPS location.

1. Visit the UPS store you selected; Call ahead and confirm they will be available to complete your fingerprints on a walk-in basis.
2. Ask for the 'livescan' fingerprinting, they will charge a fee, about $60.
3. Wait for the report/results (the results of the report will go to your address, make sure your address is correct on the form) The results come in 7-10 days.
4. Once you receive your results, make an appointment with me to discuss or complete the next step. Call me or use the scheduling link below.
